Evaluation and Management Coding & Documentation Workshop

HealthARCH provides a full day of face-to-face Evaluation and Management (E/M) Coding & Documentation training for coders/billers, practice managers and physicians/clinicians. Our certified trainers can prepare you and your practice to assess documentation according to national guidelines, understand how to apply the rules, and ensure appropriate E/M category and level assignment.

Objectives:

Documentation: The Basis of E/M Coding and Documentation Guideline Requirements

Participants will be able to:

  • Describe the purpose E/M coding
  • Summarize the importance of Documentation
  • Classify the legal responsibilities
  • Define the difference between New vs. Established patient
  • Define and explain the 1995 & 1997 Documentation Guidelines. 


E/M Coding & Documentation Key Component: History

Participants will be able to:

  • Describe/Write a valid Chief Compliant
  • Summarize and select the elements of the History or Present Illness
  • Record an appropriate Review of Systems
  • Define the two types of Past, Family & Social History


E/M Coding & Documentation Key Component: Physical Exam Key Component: Medical Decision Making (MDM)

Participants will be able to:

  • Describe/Write a Medically Necessary Physical Exam
  • Summarize and select the elements of the Medical Decision Making Component
  • Record an appropriate Number of Diagnosis
  • Determine the Amount of Date being reviewed
  • Define the Level of Risk


Medical Decision Making (MDM): Scenarios Assigning the Component and Determining the E/M code 

E/M Coding & Documentation Alternate Components:

-Time as a Factor

-Preventive Medicine

-E/M Modifiers

Participants will be able to:

  • Describe/Write the circumstances where it is appropriate to use Time as a Factor
  • Record the appropriate documentation of times for a Timed visit
  • Summarize and select the appropriate Preventive Medicine code
  • Define the appropriate use E/M modifiers
